But in the rush to deploy, most agents get broader permissions than they need because "it's easier" and "we'll tighten it later." Later never comes.

3. Audit Trails

When an AI agent takes an action, there needs to be a record. What did it do? Why? What data did it use? What was the outcome?

Without audit trails, you can't debug failures, you can't demonstrate compliance, and you can't answer the question that regulators and customers will increasingly ask: "Who's responsible for what your AI did?"
